Dental and Vision Insurance

Libraries and/or individual employees of enhanced member libraries may purchase the dental and vision insurance plans offered by SERLS.  This allows library employees to receive quality dental and vision insurance at a low cost.  Check out the PDF brochures listed below for details on coverage.  The applications for insurance are also below.  The completed application can be faxed to
